1) Human Factors GATEWAY approach: Using outputs from SCTA to support rapid identification of HF issues

Authors: Dominic Furnuss, Jamie Henderson, Karam Singh Bhinder

In this paper we introduce an approach to leverage
the output from a single SCTA to support rapid identification of HF issues in related tasks HF GATEWAY (Generic Audit Tool Exercise to Widen and Accelerate Yield). We describe a case study where we created a generic audit tool from an SCTA study, which can subsequently be used by non-HF people to widen and accelerate the HF learning to tasks of a similar nature.

2) Human Factors Issues in Turnarounds (TARs)

Authors: Jamie Henderson, Richard Marshall

As part of the planning activity for a significant TAR at a large process plant, shut-down and start-up tasks
were, over a period of several months, subjected to a form of HF Critical Task (CTA). This paper summarises and discusses the general TAR issues identified by these reviews. Areas covered by this paper include: TAR planning, plant issues (e.g. equipment identification, access), procedures, competence, isolation management, purging, gas testing, and leak testing.

3) Integration of Qualitative Human Factors Safety Critical Task Analysis with Wider Engineering Risk Assessment

This paper explores how SCTA can be integrated with MAH Risk Assessments (e.g. HAZOPS, LOPAs) so there is a closer integration between Human Factors and existing engineering approaches at site. The paper includes guidance on making ALARP judgements and illustrating the degree of credit that can be taken for human performance.
